Zusammenfassung

VitalCaring Group is seeking a Home Health Physical Therapist to help patients regain mobility and independence in their homes. You will lead evaluations, create individualized plans, and provide hands-on therapy to improve function.

You will collaborate with physicians, nurses, and therapists to ensure coordinated care, educate families, and document progress in the EMR while maintaining autonomy in a field-based setting.

Qualifikationen

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience; home health preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Aufgaben

• Deliver Patient-Centered Care: evaluate and assess functional status, mobility, strength, and rehabilitation potential; develop and implement care plans; provide hands-on therapy; educate patients and caregivers.
• Drive Outcomes & Recovery: monitor progress; support safe transitions and discharge planning; identify needs and collaborate with interdisciplinary teams; document care in EMR; participate in team meetings.
• Lead & Support: supervise PTAs per state guidelines; provide guidance to caregivers and support staff.
