Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in patients’ homes, evaluate functional status, and develop individualized plans with the care team. This role emphasizes autonomy and meaningful patient relationships in a home-based setting.

You are a graduate of an accredited PT program, licensed in your state, with 1–2 years of home health experience preferred, strong assessment skills, and excellent communication. A valid driver’s license is required.

Qualifications

• Graduate from an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience (home health preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• CPR certification current.
• Valid driver’s license with reliable transportation and auto insurance.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehab potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Document patient care and progress accurately in the EMR.
• Participate in case conferences and team meetings.
• Supervise Physical Therapist Assistants (PTAs) per state guidelines.
• Provide guidance and education to caregivers and support staff.
