Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to help patients recover, regain mobility, and maintain independence at home. You will work autonomously within the care team to deliver patient-centered therapy across Orange and Osceola Counties, Florida.

Youll evaluate functional status, develop individualized plans, supervise PTAs, educate patients and families, and document progress in the EMR.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Masters).
• Current PT license in the state of practice.
• Minimum 1a02 years of experience (home health experience preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid drivers license and transportation.

Responsibilities

• Evaluate and assess patientsunctional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety.
• Monitor progress and adjust plans; manage discharge planning.
• Document care and progress accurately in the EMR; participate in team meetings.
• Supervise Physical Therapist Assistants (PTAs) per state guidelines.
• Support and educate caregivers and staff.
