Optum's Patient Care team is seeking a traveling Physical Therapy Assistant (PTA) to deliver high-quality home care across Stamford, Greenwich, Norwalk and Wilton, CT. You will support patients in their homes, implement the plan of care, and educate families on exercises and safety.

Responsibilities include following state PT guidance, guiding active and passive exercises, recording responses, and coordinating with the supervising PT and care team.

Qualificações

• Licensed, registered or certified to practice physical therapy in CT
• Driver's license and access to a vehicle or reliable public transportation
• Available to work a weekend rotation
• CPR certification or ability to obtain within 90 days

Responsabilidades

• Follow state laws defining the provision of physical therapy in home care and act within the plan of care
• Instructs patients in active and passive exercise, gait, ADL, transfer, safety and prosthetic training
• Observe, record, and report patient responses; coordinate care with supervising PT and health care team
• Educate patients, family, caregiver, and others on therapy within PTA scope
• Function effectively in diverse home environments and with all patients
