Job summary

Atrinity Home Health is seeking a licensed Physical Therapist Assistant to provide treatment and education in home care settings under the supervision of a Physical Therapist. In this role you will deliver exercise instructions, monitor progress, and adapt plans to improve patient well-being.

You will work within a dedicated care team, following state regulations and a defined plan of care while supporting patients and their families with compassionate, skilled therapy in the home.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.

Responsibilities

• Follow state-specific laws governing the provision of physical therapy in home care and implement the plan of care as defined by the supervising Physical Therapist.
• Instructs and aids patients in active and passive exercise, muscle re-education, gait, functional activities, ADL, transfers, safety and prosthetic training.
• Observes, records, and reports to the supervising PT and care team about patient response and condition changes; coordinates care with the healthcare team.
• Instructs patient, family, caregiver, and other health team members in therapy within the PT scope.
• Function in diverse home settings regardless of age, race, creed, color, sex, disability, or financial condition.
