Job summary

St. Francis Medical Center Home Health is seeking a Physical Therapist to assess patient needs in home health, develop a treatment plan, and evaluate progress, collaborating with physicians and nurses to improve mobility and comfort.

You will join a team within the Optum family and receive competitive pay with per-visit and hourly components, plus a broad benefits package. A Louisiana license and reliable transportation are required.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws… in accordance with the plan of care, and in coordination with other health care team members.
• Observes and reports patient response to treatment and changes in condition to supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and/or maintain maximum functioning.
• Instructs and informs health care team, patient, and family/caregivers about plan of care and progress.
• Visits patients according to Plan of Treatment; completes progress notes per policy.
• Completes all patient evaluations and develops PT plan of care per state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
• Functioning in diverse home environments regardless of patient demographics.
