St. Francis Medical Center Home Health, part of the Optum family, is hiring a Physical Therapist to assess and evaluate patient care needs related to functional status and mobility in home health settings and to help determine treatment plans.

You will perform interventions to improve well‑being, document progress, and collaborate with the healthcare team while adhering to state guidelines; requires current PT licensure in the state and a valid driver’s license.

Formación

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: Current CPR certification or ability to complete within 90 days of hire.

Responsabilidades

• Evaluates the patient's functional status and physical therapy needs.
• Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and plan of care.
• Observes and reports the patient's response to treatment and changes in condition.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers regarding plan of care and progress.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
