Job summary

Louisiana HomeCare of River Parishes, part of the LHC Group and a member of the Optum family, is seeking a Physical Therapist to join our home health team. You will assess patient needs, develop a treatment plan, and deliver interventions to improve mobility and well-being while coordinating with physicians and nurses.

This role requires current Louisiana Physical Therapy licensure, a valid driver’s license, and CPR certification ideally.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and access to a dependable vehicle or public transportation.
• CPR certification preferred or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates patient functional status and needs; assists in therapy plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient response to treatment to supervising nurse/physician.
• Treats patients to relieve pain and improve function.
• Instructs patient and family on plan of care and progress toward goals.
• Visits patients according to treatment plan and documents notes.
• Completes PT evaluations and develops plan of care per state guidelines.
• Reports outcomes and projected frequency of care.
• Function in diverse home settings irrespective of patient demographics.
