St. Francis Medical Center Home Health, part of LHC Group and within the Optum family, seeks a Physical Therapist to assess patient needs, determine treatment plans, and implement interventions to improve mobility and function in home health settings.

You will evaluate progress, educate patients and families, and document visits according to plans of care. Requires state licensure, reliable transportation, and CPR certification or the ability to obtain within 90 days of hire.

Formación

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsabilidades

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within the scope of practice as defined by state laws.
• Observes and reports the patient's response to treatment and any changes in condition.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers regarding plan of care and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and frequency of care.
