Louisiana HomeCare of River Parishes, part of LHC Group and the Optum family, is seeking a Physical Therapist for home health. You will assess patients, determine the therapy plan of care, and provide interventions to improve mobility, endurance, and functional status.

You will work with a care team to monitor progress, educate patients and families, and document outcomes according to policy. A valid PT license and driver’s license are required; CPR certification is preferred.

Qualifikationen

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

• Evaluates the patient's functional status and physical therapy needs.
• Develops and implements the plan of care in coordination with physicians and the care team.
• Treats patients to relieve pain and restore function within state guidelines.
• Instructs patients and families about plan of care and progress toward goals.
• Documents visits and progress per policy; reports outcomes and goals.
