Louisiana HomeCare of River Parishes, part of the Optum family, is seeking a Physical Therapist in Home Health. You will evaluate patients, determine treatment plans, and perform interventions to improve well-being, coordinating with nurses and physicians.

Formación

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsabilidades

• Evaluates the patient’s functional status and physical therapy needs.
• Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws governing PT.
• Observes and reports patient response to treatment and changes to supervising nurse/physician.
• Treats patients to relieve pain and restore or maintain function.
• Instructs patient and family/caregivers on plan of care and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
