Qualifications

• Provides PT services within state-defined scope and plan of care.

• Educates patient and family on plan of care and progress toward goals.
• Visits patients per Plan of Treatment and documents progress per policy.
• Develops PT plan of care per state guidelines; reports outcomes and frequency of care.
