Optum's Mederi Caretenders home health team seeks a Physical Therapist to assess patient needs, develop and implement therapy plans, and monitor progress in home settings across Florida. You will collaborate with physicians and nurses to optimize mobility, pain relief, and functional outcomes.

Responsibilities include evaluating functional status, planning and delivering care within state guidelines, and communicating progress to families and care teams.

Formación

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to dependable vehicle or public transportation.
• Preferred CPR certificate or ability to obtain within 90 days of hire.

Responsabilidades

• Evaluates the patient's functional status and physical therapy needs, assists in developing the therapy plan of care.
• Provides services within the scope of practice and coordinates with health care team.
• Observes and reports patient response, documents progress and completes plan of care within guidelines.
• Delivers patient care to relieve pain, restore function, and maximize functioning.

Current Physical Therapy licensure
