Job summary

Optum is seeking a Physical Therapist in Home Health in Florida to perform assessments, develop and implement therapy plans, and evaluate patient progress. This role requires collaboration with physicians and healthcare teams to improve patient well-being and function.

Responsibilities include conducting evaluations, documenting progress notes, and delivering interventions to relieve pain and restore activity within state-specific guidelines.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and the plan of care.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patients, families, and caregivers regarding plan of care and progress.
• Completes patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
