Job summary

Mederi Caretenders, a part of LHC Group, is seeking a Physical Therapist for home health. You will assess patients’ functional status, develop treatment plans, and administer therapies to improve mobility and reduce pain in the patient’s home.

You will document progress, communicate with physicians and nurses, and adapt plans to patient needs across diverse home environments. CPR certification preferred or obtainable within 90 days of hire.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Assists in development of the therapy plan of care with physician input.
• Provides services within scope of practice and coordinates with health care team.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patients, family, and caretakers about plan of care and progress.
• Visits patients according to Plan of Treatment and submits progress notes.
• Completes patient evaluations and develops PT plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
• Function in diverse home settings regardless of client demographics.
