UnitedHealth Group's Mederi Caretenders division in Florida seeks a Physical Therapist for home health to assess patients, determine a therapy plan, and deliver interventions that improve mobility and comfort in the home setting. You will document progress, coordinate with physicians and nurses, and educate patients and families about plans of care while complying with state practice guidelines.

This role offers a pay mix including per-visit and hourly compensation, with full-time expectations

Qualificações

• Licensed Physical Therapist in the state of practice.
• Driver's license and vehicle access or reliable transportation.
• Preferred CPR certification or ability to obtain within 90 days of hire.

Responsabilidades

• Evaluates the patient's functional status and PT needs, assisting the physician in developing the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and in coordination with the health care team.
• Observes and reports the patient's response to treatment and changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Informs patient and family/caregivers about the plan of care, limitations, and progress toward goals.
• Visits patients according to the Plan of Treatment and submits progress notes per policy.
• Completes evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
• Maintains ability to function in any home situation regardless of patient demographics.
