Job summary

Elite Home Health, part of LHC Group, is hiring a Physical Therapist to assess and treat home-based patients, develop plans of care, and coordinate with the healthcare team. The role emphasizes improving mobility, endurance, pain management, and overall well-being in the patient’s home environment.

Responsibilities include evaluating functional status, implementing therapy plans, and communicating progress to nurses, physicians, patients, and families.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists the physician in developing the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and the plan of care, coordinating with the health care team.
• Observes and reports patient response to treatment and changes in condition to supervising nurse or physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning according to standards of practice.
• Instructs patient, family, and caregivers about the plan of care, functional limitations, and progress toward goals.
• Visits patients according to the Plan of Treatment; completes progress notes per policy.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
• Functions in various home settings regardless of age, race, creed, or disability.
