Job summary

Elite Home Health, a part of the LHC Group, is hiring a full-time Physical Therapist for Home Health. Coverage area Helena, AR and surrounding areas. Join a culture of Caring, Connecting, Growing together with UnitedHealth Group family.

You will assess patients, develop treatment plans, and provide interventions to improve well-being, pain relief, mobility, and function, while coordinating with the care team and documenting each visit.

Qualifications

• Current Physical Therapy licensure in Arkansas.
• Current CPR certification.
• Current driver’s license and vehicle insurance or access to a dependable vehicle.

Responsibilities

• Evaluate patient functional status and physical therapy needs.
• Develop and implement plan of care within state guidelines.
• Treat patients to relieve pain and restore function.
• Inform patient and family about plan of care and progress.
• Visit patients per Plan of Treatment and document progress.
• Complete evaluations and PT plan of care per state guidelines.
• Report outcomes and frequency of care.
