Job summary

Qualifications

• Graduate from an accredited physical therapy program per CAPTE.
• Current license to practice PT in the assigned state.
• Current CPR certification.
• At least 1 year of related PT experience in home health; less than 1 year requires mentoring approval.

Responsibilities

• Provide skilled physical therapy services in patients' homes to promote mobility and function.
• Reexaminations at minimum every 30 days and discharge planning as needed.
• Collaborate with patients, caregivers, and an interdisciplinary team to support recovery and prevent hospitalizations.
