Job summary

Caretenders Home Health, part of LHC Group and a member of the Optum family, seeks a Physical Therapist to assess and design therapy plans for home health patients in Indiana. You will evaluate functional status, implement interventions, and monitor progress in collaboration with the care team.

Responsibilities include documenting progress, educating patients and families, and ensuring plan of care adheres to state guidelines.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient’s functional status to determine therapy needs.
• Develops and implements a plan of care in collaboration with the physician.
• Provides therapy within state practice guidelines and plan of care.
• Observes and reports patient responses to treatment to supervising nurse/physician.
• Treats patients to relieve pain and restore function while maintaining safety standards.
• Educates patient and family about plan of care and progress toward goals.
• Completes visits and progress notes according to policy.
• Completes PT plan of care within state guidelines and reports outcomes.
• Adapts to various home environments and patient demographics.
