Job summary

UnitedHealth Group, through Caretenders VNA (LHC Group, Optum family), seeks a Physical Therapist for home health in Cincinnati. You will assess patients, develop and implement treatment plans, and collaborate with a health team to improve mobility, endurance, and comfort.

A focus on patient education and progress documentation is essential. The role requires PT licensure, a valid driver's license, and CPR (preferred within 90 days).

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within the scope of practice as defined by state laws and plan of care.
• Observes and reports patient responses to treatment to the supervising nurse and/or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient, family and caregivers regarding plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
