Job summary

Guardian Angel Home Care is seeking Home Health Physical Therapists (PTs) to provide patient-centered rehabilitation in clients' homes. New graduates and experienced PTs are encouraged to apply, working with a supportive leadership team across 20 offices nationwide.

As a Home Health PT you will evaluate patients, create individualized plans, and guide recovery to promote independence and prevent hospital readmissions, while benefiting from flexible scheduling and strong clinical support.

Qualifications

• Active Physical Therapist (PT) license in the state of practice.
• Graduate of an accredited Physical Therapy program (DPT or equivalent).
• CPR / BLS Certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform comprehensive patient evaluations in the home.
• Develop individualized treatment plans based on physician orders.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on home exercise programs.
• Implement fall prevention and safety strategies.
• Document patient progress in EMR systems.
• Communicate with physicians, nurses, and interdisciplinary care teams.
• Adjust treatment plans based on patient progress.

About the Role

As a Home Health Physical Therapist, you will provide individualized rehabilitation care directly in patients' homes. You will evaluate patients, develop treatment plans, and guide them through recovery while helping them regain independence and prevent hospital readmissions.

Home health allows physical therapists to:
