Job summary

Guardian Angel Home Care is seeking Physical Therapists (PTs) to provide home health services in the communities we serve. You will evaluate patients in their homes and guide rehabilitation to help regain independence and prevent hospital readmissions.

This role offers flexible scheduling, one-on-one patient care, and strong clinical support. Must have a PT license in the state, DPT or equivalent, CPR/BLS, and a valid driver's license with reliable transportation.

Qualifications

• Active Physical Therapist (PT) license in the state of practice.
• Graduate of an accredited Physical Therapy program (DPT or equivalent).
• CPR/BLS certification required.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform comprehensive in-home patient evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on home exercise programs.
• Implement fall prevention and safety strategies.
• Document patient progress in EMR systems.
• Communicate with physicians, nurses, and interdisciplinary care teams.
• Adjust treatment plans based on patient progress.

About the Role

As a Home Health Physical Therapist, you will provide individualized rehabilitation care directly in patients' homes. You will evaluate patients, develop treatment plans, and guide them through recovery while helping them regain independence and prevent hospital readmissions.

Home health allows physical therapists to:
