Guardian Angel Home Care is seeking a dedicated Physical Therapist (PT) for home health services in the Columbus, OH area. You will evaluate patients in their homes, create individualized treatment plans, and guide recovery to maximize independence.

Responsibilities include therapeutic exercises, patient education, and collaboration with physicians, nurses, and caregivers. A valid PT license, DPT or equivalent, CPR/BLS, and a driver’s license are required.

Qualifications

• Active Physical Therapist license in the practicing state.
• Graduate from an accredited Physical Therapy program (DPT or equivalent).
• CPR/BLS certification is required.
• Valid driver's license and reliable transportation.

Responsabilités

• Perform home-based patient evaluations and develop treatment plans.
• Guide patients through rehabilitation to regain independence.
• Provide therapeutic exercises and mobility training in patients' homes.
• Educate patients and caregivers on home exercise programs.
• Document progress in EMR systems and coordinate with care teams.
• Adjust plans based on patient progress and physician orders.

About the Role

As a Home Health Physical Therapist, you will provide individualized rehabilitation care directly in patients' homes. You will evaluate patients, develop treatment plans, and guide them through recovery while helping them regain independence and prevent hospital readmissions.

Home health allows physical therapists to:

Responsibilities

• Perform comprehensive patient evaluations in the home
• Develop individualized treatment plans based on physician orders
• Provide therapeutic exercises and mobility training
• Educate patients and caregivers on home exercise programs
• Implement fall prevention and safety strategies
• Document patient progress in EMR systems
• Communicate with physicians, nurses, and interdisciplinary care teams
• Adjust treatment plans based on patient progress

Minimum Qualifications

• Active Physical Therapist (PT) License in the state of practice
• Graduate of an accredited Physical Therapy program (DPT or equivalent)
• CPR / BLS Certification
• Valid driver's license and reliable transportation

Preferred Qualifications

• Home Health experience
• Familiarity with EMR documentation systems
• Strong communication and patient education skills
• Ability to work independently in the field
