Job summary

Guardian Angel Home Care seeks a Home Health Physical Therapist to deliver in-home rehabilitation, assess patients, and tailor plans to promote independence and prevent readmissions.

Qualifications

• Active Physical Therapist license in the state of practice.
• Graduate of an accredited Physical Therapy program (DPT or equivalent).
• CPR / BLS certification is required.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform home health patient evaluations and develop treatment plans.
• Provide individualized rehabilitation and therapeutic exercises.
• Educate patients and caregivers on home exercise programs.
• Implement fall prevention and safety strategies in the home.
• Document progress in EMR systems and coordinate with care teams.
• Adjust treatment plans based on patient progress and goals.
• Communicate with physicians, nurses, and interdisciplinary teams.

About the Role

As a Home Health Physical Therapist, you will provide individualized rehabilitation care directly in patients' homes. You will evaluate patients, develop treatment plans, and guide them through recovery while helping them regain independence and prevent hospital readmissions.

Home health allows physical therapists to:

Responsibilities

• Perform comprehensive patient evaluations in the home
• Develop individualized treatment plans based on physician orders
• Provide therapeutic exercises and mobility training
• Educate patients and caregivers on home exercise programs

• Communicate with physicians, nurses, and interdisciplinary care teams

Minimum Qualifications

• Active Physical Therapist (PT) License in the state of practice
