Job summary

Guardian Angel Home Care is seeking Home Health Physical Therapists (PTs) to provide in-home rehabilitation services across the communities we serve in California. You will evaluate patients, develop treatment plans, and guide recovery to regain independence while preventing readmissions. This role offers flexible scheduling, strong clinical support, and opportunities to work one-on-one with patients in real-world settings.

New graduates and experienced PTs encouraged to apply.

Qualifications

• Active PT license in the state of practice.
• Graduate of an accredited Physical Therapy program (DPT or equivalent).
• CPR/BLS certification required.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform comprehensive patient evaluations in the home
• Develop individualized treatment plans based on physician orders
• Provide therapeutic exercises and mobility training
• Educate patients and caregivers on home exercise programs
• Implement fall prevention and safety strategies
• Document patient progress in EMR systems
• Communicate with physicians, nurses, and interdisciplinary care teams
• Adjust treatment plans based on patient progress

About the Role

As a Home Health Physical Therapist, you will provide individualized rehabilitation care directly in patients' homes. You will evaluate patients, develop treatment plans, and guide them through recovery while helping them regain independence and prevent hospital readmissions.

Home health allows physical therapists to:
