Job summary

Guardian Angel Home Care is seeking a Home Health Physical Therapist to provide personalized rehabilitation in patients’ homes. You will evaluate, treat, and guide recovery to improve independence and reduce hospital readmissions.

Responsibilities include creating care plans, educating patients and caregivers, and documenting progress in EMR systems while collaborating with a broader care team.

Qualifications

• Active Physical Therapist license in the state of practice.
• Graduate of an accredited Physical Therapy program (DPT or equivalent).
• CPR/BLS certification is required.

Responsibilities

• Develop individualized treatment plans based on physician orders.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on home exercise programs.
• Implement fall prevention and safety strategies.

• Adjust treatment plans based on patient progress.

About the Role

As a Home Health Physical Therapist, you will provide individualized rehabilitation care directly in patients' homes. You will evaluate patients, develop treatment plans, and guide them through recovery while helping them regain independence and prevent hospital readmissions.

Home health allows physical therapists to:

Minimum Qualifications

• Active Physical Therapist (PT) License in the state of practice

• CPR / BLS Certification
• Valid driver's license and reliable transportation

Preferred Qualifications

• Home Health experience
• Familiarity with EMR documentation systems
• Strong communication and patient education skills
• Ability to work independently in the field
