Job summary

Guardian Angel Home Care is seeking a Home Health Physical Therapist to provide in-home rehabilitation services, assess patients, and develop personalized treatment plans. You will guide patients toward independence and reduce hospital readmissions in a supportive home health setting.

You will work with a team that emphasizes patient outcomes, clinician support, and flexible scheduling options, ideal for PTs seeking autonomy and field-based practice.

Qualifications

• Active Physical Therapist license in the state of practice.
• Graduate of an accredited Physical Therapy program (DPT or equivalent).
• CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive patient home evaluations and develop individualized treatment plans.
• Provide therapeutic exercises and mobility training in patients’ homes.
• Educate patients and caregivers on home exercise programs.
• Document patient progress in EMR and communicate with physicians and interdisciplinary teams.
• Adjust treatment plans based on patient progress.

About the Role

As a Home Health Physical Therapist, you will provide individualized rehabilitation care directly in patients' homes. You will evaluate patients, develop treatment plans, and guide them through recovery while helping them regain independence and prevent hospital readmissions.

Home health allows physical therapists to:

Responsibilities

• Perform comprehensive patient evaluations in the home

• Document patient progress in EMR systems
• Communicate with physicians, nurses, and interdisciplinary care teams
• Adjust treatment plans based on patient progress

Minimum Qualifications

• Active Physical Therapist (PT) License in the state of practice
• Graduate of an accredited Physical Therapy program (DPT or equivalent)
• CPR / BLS Certification
• Valid driver's license and reliable transportation

Preferred Qualifications

• Home Health experience
• Familiarity with EMR documentation systems
• Strong communication and patient education skills
• Ability to work independently in the field
