Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health to assess patients' functional status, mobility, endurance, and pain, and to develop and implement a plan of care in collaboration with the care team.

You will visit patients in home settings, document progress, adhere to state practice laws, and coordinate with nurses and physicians to optimize patient well-being. A valid PT license and CPR are required.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance or access to a dependable vehicle.
• Ability to function in any home situation regardless of client background.

Responsibilities

• Evaluates patient functional status and PT needs; assists in therapy plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient response and condition changes.
• Treats to relieve pain, restore function, and maintain maximum functioning.
• Informs patient and family about plan of care and progress.
• Visits patients per Plan of Treatment and documents progress notes.
• Completes evaluations and develops PT plan of care per state guidelines.
• Reports outcomes, goals, and projected frequency of care.
