Job summary

Providence at Home with Compassus seeks a licensed Physical Therapist to deliver in-home care, assess and treat diverse diagnoses, and guide families in continued therapy. You will work under a physician's plan of care in patients' homes, promoting independence and functional improvements.

A strong focus on compassionate, patient-centered care is essential. Required are PT licensure within 30 days of hire, and advanced degrees (Bachelor's/Master's/Doctorate) in Physical Therapy, plus BLS and AHA

Qualifications

• Bachelor's, Master's and Doctorate degrees in Physical Therapy from accredited programs.
• Current, unencumbered Physical Therapist license within 30 days of hire.
• National Provider BLS and American Heart Association (Vendor Managed) certifications.

Responsibilities

• Plan and administer medically prescribed PT treatments to restore function and reduce pain.
• Perform comprehensive physical assessments and determine needs for referrals.
• Instruct patients and families for home-based continued care; supervise PT aides.
