Job summary

Providence at Home with Compassus is seeking a licensed Physical Therapist Assistant to provide hands-on therapy in patients’ homes under a supervising PT. You’ll implement treatment plans, document progress, and communicate with the care team to ensure safe, effective care.

The role requires autonomy across age groups, strong documentation skills, and collaboration with an interdisciplinary team. An associate degree in physical therapy is required, with home-health experience preferred.

Qualifications

• Current, unencumbered PTA license required.
• Preferred 1 year of PTA home health or rehab experience.
• Associate degree from an accredited PTA program.

Responsibilities

• Deliver quality therapy by implementing PT plans and documenting progress with supervising PT.
• Demonstrate competency across age groups and identify needs for referrals.
• Maintain timely documentation meeting agency standards and billing requirements.
• Recommend follow-up after home health, infusion, or hospice services.
• Adapt techniques/equipment to maximize safety and function at home.
• Independently manage caseload under PT supervision and schedule visits.
• Coordinate with interdisciplinary team and transfer information as needed.
• Participate in training to stay current and maintain licensure.
• Meet productivity standards and orient new team members.
