Job summary

Advocate Health At Home is seeking a licensed Physical Therapist to manage a patient caseload in home health, ensuring timely care and documentation per physician orders. The PT will collect assessments, analyze data, and coordinate with a multidisciplinary team to optimize patient outcomes and safety.

Responsibilities include developing and implementing care plans, performing PT services, and maintaining clear communication with physicians and supervisors while adhering to regulatory guidelines

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• State Physical Therapy License required.
• Basic Life Support for Healthcare Provider certification from the American Heart Association required.

Responsibilities

• Manage daily patient caseload and documentation per plan of care.
• Collect data from patient assessments and EMR information in home/telehealth settings.
• Coordinate care with multidisciplinary clinical team to optimize outcomes.
• Provide PT services certified by the patient’s physician and ensure proper plan of care implementation.
• Maintain ongoing communication with the team and update schedules and orders.
