Job summary

Advocate Health at Home is hiring a licensed Physical Therapist to manage a home health patient caseload, coordinating care with physicians and a multidisciplinary team. You will assess patients, collect and analyze data, and ensure documentation aligns with plans of care and regulatory guidelines.

The role requires a Doctorate in Physical Therapy, an active state license, BLS, and the ability to travel to patient homes.

Qualifications

• Doctorate degree in Physical Therapy from an accredited school.
• Current Physical Therapy license in the applicable state; BLS certification.
• Valid driver’s license and auto insurance at all times.

Responsibilities

• Manage daily patient caseload with timely care and documentation.
• Collect data from patient assessments and EMR to inform care.
• Identify high-risk patients and coordinate interventions to reduce re-hospitalization.
• Develop and implement plan of care in collaboration with the patient’s physician.
• Coordinate multidisciplinary team and supervise LPTAs and HHAs involved in care.
• Maintain communication with the physician and branch supervision regarding changes in patient conditions.
