Job summary

Advocate Health seeks a dedicated Physical Therapy Assistant (PTA) to manage caseloads, collect data, and implement therapy plans in home health settings. You will document care, communicate with the multidisciplinary team, and ensure policies and standards are met while supporting patient discharge planning.

Required PTA licensure and BLS; 1 year of clinical or home health experience is preferred. Travel to patient residences is expected.

Qualifications

• Graduate of an approved PTA program (required).
• Current state licensure as a PTA (required).
• Maintain current state driver's license and auto insurance at all times.
• 1-year experience as licensed PT in clinical care or home health (preferred).
• BLS for Healthcare Providers (required).
• Pedriatics experience (preferred) or related training.

Responsibilities

• Manage assigned patient caseload daily with timely care and documentation.
• Collect health data from home assessments and EMR.
• Implement care programs including modalities, procedures, exercise, and education.
• Document and execute plan of care addressing patient/family needs and discharge planning.
• Observe, record, and report treatment reactions or condition changes to physician/PT.
• Communicate updates on schedules, patient data, and orders with multidisciplinary team.
• Ensure clear documentation per policy and standards; meet productivity targets.
• Perform treatments per PT Practice Act, Home Health Conditions, and agency policy.
• Promote quality practice and integrate evidence into care.
