Job summary

Advocate Health is seeking a licensed Physical Therapist to join our Home Health team as a Case Manager. You will manage your own caseload, collaborate with physicians, nurses, and interdisciplinary staff, and provide in-home therapy to diverse patient populations.

The role emphasizes independence within Advocate’s integrated healthcare system, with opportunities to improve mobility, safety, and functional independence in patients’ homes while maintaining strong professional relationships.

Qualifications

• Licensed Physical Therapist in Illinois (or eligible for licensure).
• Minimum of one year of Physical Therapy clinical experience or diverse clinical affiliations.
• Proficiency with home-based therapy equipment and assistive devices.

Responsibilities

• Travel extensively within assigned geographic boundaries.
• Work rotating weekends and holidays based on patient needs.
• Attend mandatory informational, educational, and branch meetings.
• Assess, plan, implement, and evaluate physical therapy treatments for home health patients.
• Facilitate patient progression toward self-care and independence.
