Job summary

Caretenders, a home health provider, seeks a Physical Therapist to perform patient assessments and develop treatment plans focused on mobility and functional status. You will evaluate progress, deliver interventions, and collaborate with nurses and physicians to optimize outcomes in the patient’s home.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to dependable vehicle or public transportation.
• CPR certification preferred (not required)

Responsibilities

• Evaluates patient functional status and PT needs; assists physician in therapy plan development.
• Provides PT services within scope of practice and plan of care; coordinates with health-care team.
• Observes and reports patient responses to treatment and changes in condition to supervising staff.
• Treats patients to improve and restore function and maintain maximum functioning.
• Instructs patients and families regarding plan of care, limitations, and progress.
• Visits patients per Plan of Treatment; documents progress notes and completes evaluations.
• Develops PT plan of care within state guidelines; reports outcomes and projected frequency of care.
