Qualifications

• BS in Physical Therapy and current PT licensure.
• Two years’ PT experience in acute care or rehab setting.
• Valid driver's license and CPR certification.

Responsibilities

• Provide one-on-one PT services in patients' homes.
• Develop and implement individualized home health plans of care.
• Coordinate with patients' families and care teams to support daily living activities.
