Compassus is seeking a Home Health Physical Therapist to assess, plan, implement, and evaluate patient care to restore or maintain well‑being. You will coordinate with the interdisciplinary team and document progress in patient records.

Qualified candidates hold a Bachelor of Science in Physical Therapy, an active license, CPR certification, and about two years’ experience in acute care or rehabilitation. You will teach transfer techniques, device use, and participate in ongoing professional

Qualificações

• Bachelor of Science in Physical Therapy required.
• Two years of PT experience in an acute care or rehabilitation setting highly preferred.
• Active and unencumbered Physical Therapist license in state of employment; CPR certification required.

Responsabilidades

• Provides direct patient care following physician orders and clinical assessment.
• Completes Plan of Care in consultation with physician and care team; documents progress notes and orders.
• Performs initial and ongoing assessments including OASIS where applicable; determines patient eligibility for home health.
• Performs and supervises care by team members according to patient needs.
• Educates patient and family on transfer techniques and safe use of equipment.
