Job summary

Compassus seeks a licensed Home Health Physical Therapist to provide coordinated care to patients in a home-health setting. You will plan, implement, and evaluate treatment plans based on medical referrals and assessments.

The role requires a BS in Physical Therapy, active licensure, CPR certification, and preferably 2 years of experience in acute care or rehabilitation. Collaboration with an interdisciplinary team is essential to deliver high-quality care.

Qualifications

• BS in Physical Therapy is required.
• Active Physical Therapist license in state(s) of employment is required.
• Current CPR certification is required.

Responsibilities

• Provide home health physical therapy per plan of care and physician orders.
• Perform initial and ongoing assessments (including relevant scales) to determine patient needs and level of functioning.
• Develop, implement, and revise Plan of Care in collaboration with the physician and care team.
• Educate patients and families on transfer techniques and safe use of equipment.
