Job summary

CHRISTUS HomeCare, part of LHC Group and in the Optum family, seeks a dedicated Physical Therapist for Home Health to assess patients' functional status, determine a plan of care, and implement interventions to improve mobility and well-being. You will document progress, collaborate with physicians and nurses, and ensure treatment adheres to state guidelines.

A valid PT license and CPR certification are required; driving and reliable transportation are expected.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and PT needs, assisting with plan of care.
• Provides services within the scope of practice as defined by state laws and in coordination with the care team.
• Observes and reports patient responses to treatment to the supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family regarding the plan of care and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes all evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation and anticipated frequency of care.
• Function effectively in any home setting regardless of patient background.
