Job summary

CHRISTUS HomeCare in Texas, part of the LHC Group within the Optum family, is seeking a Physical Therapist in Home Health. You will complete assessments and evaluations of functional status, mobility, endurance, and pain to help shape a treatment plan.

You will provide therapy interventions to improve and restore function, document progress, and coordinate with the supervising nurse and physician while adhering to state guidelines and the plan of care.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient’s functional status and assists in therapy plan of care.
• Provides services within scope of practice and in coordination with care team.
• Observes and reports patient responses to treatment to supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, maintain max functioning.
• Instructs patient and family regarding plan of care and progress.
• Visits patients per Plan of Treatment and submits progress notes.
• Completes patient evaluations and develops PT plan of care per state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
• Function in home settings regardless of age, race, etc.
