Job summary

At Home Healthcare, a part of LHC Group, is seeking a dedicated Physical Therapist to conduct home health assessments, determine treatment plans, and perform interventions aimed at improving patients' mobility and well-being. You will evaluate progress and adapt care to each patient’s needs, working with the health care team to ensure quality outcomes.

Responsibilities include conducting assessments, developing PT plans of care within state guidelines, reporting outcomes, and educating patients

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within the scope of practice and plan of care.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain and restore function.
• Instructs patients and families regarding plan of care and progress toward goals.
• Completes patient evaluations and documents progress notes per policy.
