Job summary

CHRISTUS Home Health, part of the Optum/LHC Group family, seeks a licensed Physical Therapist to perform home health evaluations and develop treatment plans aimed at improving mobility and reducing pain. You will document progress and collaborate with nurses and physicians to coordinate care.

The role requires state licensure, a valid driver's license, and reliable transportation. CPR certification is preferred and may be obtained within 90 days of hire.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred (can complete within 90 days of hire)

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within the scope of practice as defined by state laws and plan of care.
• Observes and reports patient response to treatment to supervising nurse/physician.
• Treats patients to relieve pain and restore function.
• Instructs patient and family regarding plan of care and progress.
• Visits patients according to Plan of Treatment and documents progress notes.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
