Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health to assess patient needs, develop treatment plans and improve mobility and function in the home setting across Florida. You will collaborate with physicians, nurses and families to deliver high-quality care and document progress per state requirements.

Qualifications

• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs and assists the physician in developing the therapy plan of care.
• Provides services within the scope of practice and coordinates with the health care team.
• Observes and reports patient responses to treatment and changes in condition.
• Treats patients to relieve pain, restore function and maintain maximum functioning.
• Instructs patients and families regarding plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes evaluations and develops PT plan of care per state guidelines.
• Reports outcomes and frequency of care.
• Function in diverse home settings with respect to all patients.
