Job summary

AdventHealth is seeking a Licensed Physical Therapist to deliver high-quality PT care in the home health setting in Florida. You will evaluate patients, develop and implement individualized care plans, and educate families on therapies and assistive devices.

The role requires a Doctorate in Physical Therapy (preferred) with active Florida PT licensure, BLS, and a valid driver’s license. Join a team focused on compassionate, patient-centered care and strong community impact.

Qualifications

• Doctorate in Physical Therapy (preferred).
• Licensure as Physical Therapist (PT) in Florida (Required).
• Basic Life Support/ CPR certification (Required).
• Driver's License (Required).
• Auto Insurance (Required).

Responsibilities

• Develops professional and clinical skills through staff development meetings and affiliations.
• Admits new patients and performs comprehensive initial evaluations.
• Conducts home safety evaluations and communicates needs to physicians and team.
• Establishes a plan of care with goals and implements PT assessment and care plans.
• Instructs patients and families in PT programs and use of assistive devices.
• Participates in team processes and conferences to develop patient care plans.
• Supervises home health aide activities as needed and coordinates duties.
• Maintains clear clinical records and participates in QA and training.
• Demonstrates effective communication with team, patients, and families.
• Maintains flexibility for weekend coverage and on-call rotation.
• Performs other duties as assigned.
