Qualifications

• Bachelor's or Master's degree in Physical Therapy from an accredited college.
• Licensed Physical Therapist issued by the state where you will be working.
• Minimum of one year healthcare experience.
• Current driver's license and ability to spend ~20% of your day driving to/from patient locations.
• Commitment to meet deadlines for charting and to self-manage time/schedule.
• Experience with tablets, mobile phones and EMR software.

Responsibilities

• Deliver and manage patient care plans in collaboration with the care team.
• Provide high-quality home health physical therapy services to patients in St Johns and surrounding areas.
• Participate in team-based care to ensure effective patient outcomes.
