Graham Healthcare Group is recruiting a Home Health Physical Therapist for a position in Stevensville, Michigan. The role involves managing and delivering patient care plans within a team-based home health setting.

We seek a licensed Physical Therapist with at least one year of healthcare experience, capable of driving to patient locations, and proficient with EMR software. Competitive pay and comprehensive benefits are offered.

Qualifications

• Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required.
• Licensed Physical Therapist issued by the state in which you will be working.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• The skills needed to self-manage your time and schedule.
• Demonstrated experience with tablets, mobile phones and EMR software.
