Job summary

Comprehensive Home Health Solutions is seeking a dedicated Physical Therapist to join our home health team in Northern Nevada (Reno, Sparks, Carson City and surrounding areas). You will evaluate patients in their homes, develop care plans, and guide families through prescribed exercises to restore mobility and independence.

Responsibilities include conducting assessments, collaborating with the interdisciplinary team, and ensuring accurate documentation.

Qualifications

• Current Physical Therapist license in Nevada.
• Minimum 1 year of home health PT experience preferred.
• Strong clinical, communication, and problem-solving skills.
• Ability to work independently and manage a flexible caseload.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct patient assessments, create and update care plans, and establish clear, measurable goals.
• Teach patients and families exercises and self-care techniques to improve mobility and safety.
• Perform home safety evaluations and recommend modifications when necessary.
• Work with physicians, nurses, and the care team to support high-quality outcomes.
• Ensure all visit notes and documentation are accurate and timely.
• Join case conferences and update the care team on patient progress.
• Explain care goals and progress to patients and families.
