Qualifications

• Current Physical Therapist license in Nevada.
• Minimum 1 year of home health PT experience (preferred).
• Strong clinical, communication, and problem-solving skills.
• Ability to work independently and manage a flexible caseload.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct patient assessments and create/update care plans with measurable goals.
• Teach patients and families exercises and self-care techniques.
• Perform home safety evaluations and recommend modifications.
• Collaborate with physicians, nurses, and the care team for outcomes.
• Complete visit notes and documentation accurately and on time.
• Join case conferences and update the care team on progress.
• Explain goals and progress to patients and families.
