Job summary

Comprehensive Home Health Solutions in Northern Nevada (Reno, Sparks, Carson City and surrounding areas) seeks a dedicated Physical Therapist (PT) to join our compassionate home health team. You will assess patients, develop and update care plans, and restore mobility and independence in patients’ homes.

You will work with physicians and nurses, document visits accurately, participate in case conferences, and help patients understand goals and progress while delivering safe, effective home-based

Qualifications

• Current Physical Therapist license in Nevada (required).
• Minimum 1 year of home health physical therapy experience (preferred).
• Strong clinical, communication, and problem-solving skills.
• Ability to work independently and manage a flexible caseload.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct patient assessments, create and update care plans, and establish clear, measurable goals.
• Teach patients and families exercises and self-care techniques to improve mobility and safety.
• Complete home safety evaluations and provide modification recommendations.
• Collaborate with physicians, nurses, and the interdisciplinary team to support outcomes.
• Ensure all visit notes and documentation are completed accurately and on time.
• Join case conferences and update the care team on patient progress.
• Help patients and families understand care goals, expectations, and progress.
